Licensed Practical Nurse (LPN) - Home Health Contract

Sunbelt is seeking a Licensed Practical Nurse for a Home Health contract position with a local client near Lexington, KY. Home Health LPNs with at least one year of recent home health experience are encouraged to apply for this opportunity. Responsibilities:

• Utilize expert knowledge of Medicare/Medicaid Documentation
  
• Complete accurate and detailed documentation within a 24-48-hour turnaround time
  
• Case manage at least 5-7 patients per day and 25-30 patients per week
  
• Scheduled visits will include, but are not limited to:
  
  
  
  • Providing wound care
    
  • IV administration and teaching
    
  • Cardiac program follow up and intervention
    
  
  
  
• Drive an average of 200 miles per week in the Lexington, KY area
  

Experience:

• At least one year of Home Health experience within the last year required
  
• In-depth knowledge and field experience of Medicaid/Medicare
  
• Experience case managing at least 5-7 patients per day and 25-30 per week
  

Education:

• Kentucky Licensed Practical Nurse License
  
• American Heart Association Basic Life Support
  

Employment Details:

• Employment Type: Full-time, Contract
  
• Work Hours: 8:00AM-5:00PM, 40 hours per week, some on-call and weekends required
  
• 13 week assignment with possible extension
