JOB SUMMARY


The Clinical Coordinator is responsible for the provision and coordination of care for patients (inpatient and outpatient).


American Nursing Association and other national site-specific standards will be followed, as well as established NCH Patient Navigation Policies.


In addition, the Clinical Coordinator will provide educational expertise in the field of maternal fetal medicine to clinical professional staff and the community to improve patient care skills and knowledge to ultimately enhance patient treatment outcomes.


This position requires theoretical and practical knowledge in adult education, clinical nursing and leadership skills to facilitate the delivery of quality care.


Provides educational consultation to patient with suspicion or diagnosis of high-risk pregnancy and fosters relationships with healthcare providers in the community to bridge gaps in services.


The main goals of patient navigation are to save lives, eliminating barriers to care ensuring accessibility and availability to timely delivery of services.


ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Consistent Care Coordinator : supporting patients in need of assistance for physical, psychological and social needs with one-on-one contact from any point in the continuum of care from notice of pregnancy risks and / or treatment and recovery.

  • Timely Nurse Triage : Assesses patient needs and provides educational resources to patient and family answering questions and addressing concerns during the pregnancy.


Facilitates physician recommendations for patient care to ensure patient compliance and understanding. Focuses on achieving outcomes within effective and realistic time frame.



  • Nurse Educator : to other clinical personnel to understand high-risk pregnancy and potential complications. Educator providing the facts-no opinions and not treatment advice, rather explaining advantages and disadvantages and leaves final decisions to the patient and her / his physician, supporting decisions for the patient.

  • Documentation of Navigation and Patient Satisfaction : Maintains patient case log with navigation notes and progress through continuum of care.


Documents the assistance and monitoring of patients who are navigated through the health care system monitoring efficiencies and effectiveness of providing education and needed services across all product and service lines to ensure proper access to care, guidance and support throughout the process.


Reviews patient satisfaction with practice and system services.



Eliminator of Barriers to Care : Works within the organization and through external services to identify and eliminate barriers to health care to ensure availability of services and accessibility of care services.



Assists in increasing access to culturally appropriate, supportive care when and where possible.



Quality of Care Provision : Ensures that quality, confidentiality and professionalism are threaded throughout all aspects of care and programming.



Inherent in health patient navigation is continuous quality care for patients from presentation of risk in pregnancy up to 3 months post-partum, based on the following tenets : Compassion, empathy, culturally competent care, confidentiality, respect, and patient safety.





Serves as liaison with community resource, such as various pregnancy support organizations. Supplier of and assures that current educational materials specific to health and pregnancy are available to the community upon request.


Provides educational information and support to all women concerned about their health.



Visibility and Relationship Building : Provides an open climate and flexible schedule to promote communication with patients, family members, community healthcare providers and NCH staff and providers.



Provides current and appropriate pregnancy information as requested by patients, family members and staff.



Program Evaluation of Structure, Process and Outcomes : In collaboration with the OB / GYN and pediatrics divisions, evaluates patient outcomes.



Assures forms, policies, procedures are current in assigned areas and keeps the team informed of clinical and operational issues.



A nurse Researcher : Responds to all requests for consultation in a timely manner Completed assignments in a reasonable amount of time and shares, researches and interprets the latest information on changes in health and care to heal keep staff, health care providers, patients and family members up-to-date.



Remains knowledgeable about women’s health and pregnancy related risks.



Program Financial Viability : Identifies methods to improve financial status of the maternal fetal medicine program. Evaluates and changes programs and / or services of the patient navigation program in response to changing patient population needs, new technology and organizational goals.



Evaluates the cost-effectiveness of the maternal fetal medicine patient navigation program on an on-going basis.


EDUCATION, EXPERIENCE AND QUALIFICATIONS



  • Minimum of Bachelor’s Degree in Nursing required.

  • Must have licensure as Registered Nurse in the State of Florida.

  • Minimum of 2 years of experience as an obstetrical nurse or Patient Educator required.

  • Demonstrates competence in applying teaching-learning theories to the development, implementation and evaluation of educational experience designed for the public

  • Practices health behaviors consistent with health promotion as a role model for the public.

  • Basic Life Support (BLS) certification required from the American Red Cross or American Heart Association.

  • Intermediate computer knowledge : Uses Microsoft Word, Excel, Outlook, and Windows
