Under the supervision of the Director of Outpatient Behavioral Health, the Outreach and Education Manager plays a crucial role in expanding our reach, building community partnerships, and raising awareness about the services offered at Alzheimer’s Family Center. This individual will strategically build program referrals through community contacts, physicians, and health service partnerships, and provide a strong referral foundation to support existing and future programs. Additionally, they will design and implement educational initiatives to empower caregivers, families, and the broader community with knowledge about dementia care. Duties include developing and/or maintaining mutually beneficial relationships with targeted community-based organizations, healthcare agencies, healthcare professionals, offices of elected officials, and businesses. Additionally, this individual is responsible for the co-development of the annual outreach plan to generate inquiries, referrals, and admissions and develop high-level educational sessions per the marketing plan that will promote AFC as the experts in the field of dementia services.

DUTIES:

  1. Co-design, implement and execute a strategic, focused, and comprehensive plan to recruit regular and ongoing referrals for Adult Day Health Care (ADHC) and Behavioral Health (BH) Programs, monitor accomplishments against goals.
  2. Cultivate relationships with new and existing referral sources for ADHC and BH programs, as well as new and future programs.
  3. Conduct a minimum of 120 contacts per month with referral sources, including hospitals, facilities, physicians/physician groups, and other potential referral sources, 75 of which are by direct, personal contact.
  4. Develop and maintain a regular call cycle for priority and preferred referral sources within the geriatric, dementia, and psychiatric community.
  5. Build and maintain a sales tracking database.
  6. Manage education and outreach activities according to the marketing plan. Coordinate and help promote AFC education activities.
  7. Maintain accurate records of outreach activities, partnerships, and educational programs and provide data for the generation of reports, as requested, for the board of directors or purposes such as grant applications and reports. 
  8. Assist with the planning and execution of partner events as needed (i.e., Mind Booster, Spirituality Conference, etc.). Serve as a leader or facilitator for interventions, programming, and education.
  9. Participates in the development of new programs, curriculum, materials development, and grant administration.  
  10. Work closely with the marketing team to develop promotional materials and campaigns. Utilize social media and other communication channels to share success stories, educational content, and updates about the center.
  11. Ensure flyers and other promotional materials specific to education and outreach activities are developed and in stock.
  12. Ensures that all education and outreach activities are marketed in a timely, effective manner that produces maximum attendance.
  13. Direct and engage volunteers, who support this position, and provide ongoing training and support to ensure quality service on behalf of volunteers.
